2. Form filling format :

• Full Name : ( please write your full name )
• E-Mail : ( your active e-mail is required )
• Title : please fill with “Internasional Conference Sound of Borobudur 2021”
• Work Unit : ( Your profession/job)
• Phone: In Indonesia, the initial code is +62 if you are from another country please adjusts with the country code. You don’t need to fill with the number ZERO (0) in the beginning because there is already a code +62 or use your country code.
• Company : ( your Institution name, community, group, etc.)
• ID Card / Passport : ( please upload or drag your ID Card or Passport photo to the table provided )
• Signature : ( Digital signature, if you are using a mobile phone just touch the button provided and create your digital signature by pointing your finger at the screen )

1. Next, click the yellow button “ WRITING REGISTRATION” ( for online registration-follow the event via zoom) and you will get an auto report/reply to your email after it is approved by admin.
2. If you will follow the event offline, after clicking on the REGISTRATION later you will get the OFFLINE FORM. Please fill the form same as THE FIRST FORM, which distinguishes later in the form below you will be given a choice about you have an accommodation or not. ( Note – If you do not follow the event OFFLINE, you just need to fill the first form only)

• Work Unit :……(silahkan ditulis profesi Anda, atau Anda saat ini bekerja sebagai apa)
• Phone : …. (jika Anda berasal dari Indonesia, gunakan kode awal +62, pengisian tidak perlu menggunakan angka NOL (0) di depan karena sudah ada kode +62. Jika Anda berasal dari negara lain, silahkan masukkan kode negara Anda dan nomor telpon pada kolom tersebut)
• Company:……(silahkan tuliskan nama lembaga Anda, organisasi, komunitas, dll)
• Foto Ktp atau paspor:…….(silahkan untuk mengambil foto KTP atau Paspor, atau gunakan foto KTP/paspor yang sudah ada untuk di input/drag ke kolom yang tersedia)
• Signature : …….(Tanda tangan digital) jika Anda menggunakan Mobile Phone silahkan sentuh kolom yang tersedia, dan buatlah tanda tangan digital Anda dengan menggerakkan telunjuk pada layar).

1. Berikutnya KLIK pada tulisan REGISTRASI warna kuning (untuk pendaftaran online-mengikuti acara via zoom), dan Anda akan mendapatkan report/reply secara otomatis ke email Anda, setelah di-Approval oleh admin.
2. Jika Anda ingin mengikuti acara secara Luring/ Offline, setelah KLIK pada tulisan REGISTRASI, selanjutnya Anda akan masuk pada kolom FORM OFFLINE, silahkan diisi seperti pada pengisian FORM PERTAMA, yang membedakan adalah di kolom bagian bawah, Anda akan diberi pilihan menggunakan Akomodasi atau tidak. (Catatan – Jika anda tidak mengikuti acara secara OFFLINE/LURING, anda cukup mengisi data pada FORM awal saja).
